There are TWO different PETITIONER QUESTIONNAIRES (choose only one). The selection depends upon whether or not the marriage to be evaluated by the Tribunal was a VALIDATION. A Validation occurs when either party is Catholic; marry outside of the form required by the Catholic Church, then at a later date present themselves to the church for a Catholic marriage.
